What to Expect During Your First Appointment at NCPD
Your first appointment with Dr. Andrew Bock should only take about an hour. The goal of your initial appointment is to have a thorough consultation and examination. We look over your patient records, talk with you about your treatment goals, answer any questions that you might have, and work with you to develop a personalized treatment plan that is designed to help you achieve the best possible treatment outcome.
If necessary, we might also take a set of diagnostic images at this time to help us determine the most appropriate treatment recommendations for you.
What to Bring to Your First Appointment
For your first appointment, we ask that you download and complete the new patient forms available on our website. This information helps us ensure you receive thorough care that’s tailored to your individual medical needs and concerns.
It is also helpful if you come prepared with a list of any current prescription and over-the-counter medications and/or supplements that you take.
Additionally, if you have been referred to our office by a primary physician or dentist, we ask that you request that they complete the patient referral form and send us a copy of any relevant patient records. These can either be sent directly to our office or you can bring them with you to your first appointment.
